Only fully insured large group plans must cover IVF: three cycles, lifetime. Small group, individual, and self-funded plans are exempt, and started cycles count toward the limit even if cancelled. Working out which rules apply to you is the hard part, so Sunfish checks your coverage for you and your Planning Partner walks through what's covered and what isn't, including cases where medications are covered but cycles aren't. The IVF Success Program then covers the treatment that comes after. Terms apply.
New York State runs an Infertility Reimbursement Program for eligible residents at participating clinics, and RMA of New York participates, so you apply through the clinic. Private foundations offer grants too, but they're competitive and rarely fund a full treatment plan. Yes. The IRS treats IVF as a qualified medical expense, so HSA and FSA dollars apply to treatment, medications, and storage. FSA limits reset each January, so a cycle spanning two plan years can use two years of contributions. Your Planning Partner times payments to match.
Some do, voluntarily. Self-funded employers sit outside the state mandate but often add fertility benefits anyway. Ask HR whether your plan is self-funded, because the answer decides whether New York's mandate applies to you. Sunfish builds your financial plan around either answer.
