- Eligibility for the Gold Package is determined based on patient age and Anti-Müllerian Hormone (AMH) level and is subject to confirmation prior to enrollment. Patients who meet eligibility criteria and whose first egg freezing cycle results in fewer than six (6) mature eggs retrieved may be eligible for one (1) additional egg freezing cycle at no additional cost. If eligible, the additional cycle must be initiated and completed within twelve (12) months of completion of the initial cycle. Eligibility for the additional cycle is subject to medical review, compliance with program requirements, and completion of the initial cycle in accordance with applicable clinical and program guidelines. No cash value or refunds are provided.
- All interested participants in the Sunfish Egg Freezing Success Program must complete a required physical evaluation by an eligible Fertility Clinic.
