Disadvantages of IVF Refund Guarantee Programs: The Fine Print to Read First (And How Sunfish Compares)

New to this? Start with what an IVF refund program is.

The disadvantages of IVF refund guarantee programs rarely show up in the headline refund number; they sit in the fine print that decides what counts, what’s excluded, and when money actually comes back. However, this isn’t a reason to avoid these programs; it’s a reason to read the fine print closely.

If you’re at a point where you’re weighing an IVF program, you’re likely worrying about a lot already, like the cost, the waiting period, and the paperwork. Needing some protection through the process makes complete sense.

This guide walks through six clauses found across many refund programs. Then we hold our own program to the same standard, in plain language.

Key takeaways

  • A refund program does not guarantee a baby. It defines when some money may come back.
  • The program’s definition of “success”, such as pregnancy or a live birth, decides whether a refund applies.
  • Medication, screening, and genetic testing often sit outside the program price.
  • Many refunds are partial and are worked out from the program fee only.
  • Sunfish publishes its key terms up front, including a guaranteed partial refund of up to $15,000¹ on the Gold Program. Terms and conditions apply.

Why the fine print matters more than the refund number

Refund programs, sometimes called shared-risk, tend to follow a similar pattern. You pay a higher price up front for a set number of treatment attempts. If you meet the program’s definition of success, the provider keeps the fee. If you don’t, some of it may come back.

The American Society for Reproductive Medicine (ASRM) Ethics Committee has published an opinion on these programs. Programs should define success in advance, explain costs and trade-offs clearly, and tell patients the program does not guarantee pregnancy or delivery. You should make sure to check these in any contract.

Six clauses to read before you pay

1. How “success” is defined

Some programs define success as a clinical pregnancy while others use a live birth. Under a pregnancy-based definition, a provider may keep the fee even if the pregnancy does not result in a live birth.

The ASRM Ethics Committee also notes that people who succeed in an earlier cycle may pay more than they would have under per-cycle pricing.

2. What counts as a cycle or attempt

“Three cycles” can mean different things. Some programs count only egg retrievals while others count a cancelled cycle as a used attempt. Frozen embryo transfers may or may not count toward the total. Ask how each is counted, and get the answer in writing.

3. Entry gates, and conditions that end eligibility

Many programs set entry rules, such as age limits or test results. CDC’s national assisted reproductive technology (ART) data show that outcomes vary widely with age, and many programs’ entry rules reflect that.

Some programs also list conditions that can end refund eligibility partway through:

  • Withdrawing from the program early
  • Skipping or declining a required step, such as a scheduled monitoring appointment or a recommended procedure
  • A medical change the program treats as disqualifying

Check in the contract if the provider is able to end eligibility, and on what terms.

4. What sits outside the price

Refund programs often price the treatment cycle only. The ASRM opinion notes that screening and medication costs are typically excluded. Genetic testing, embryo storage, and care for complications may also sit outside the price. Our guide to what IVF packages usually include explains what’s usually billed separately, and the same check applies when you compare IVF package deals.

5. Partial vs. full refund

Many programs refund a percentage of the program fee. That means the money you spend on medication and testing usually isn’t part of the math. Make sure to look for the exact refund formula, and ask for a worked example.

6. Deadlines and time limits

Programs often set a window to complete cycles, use frozen embryos, or request a refund. The protection may end if you miss the window, so you should note every date in the agreement before you start. For the steps involved in requesting a refund, see how to get an IVF refund.

Sunfish’s fine print, in plain language

The same six checks apply to us, so here are Sunfish’s published terms.

A fertility doctor walking a patient through program terms in a clinic office

Sunfish’s IVF and egg freezing Success Programs are packages you buy from Sunfish for treatment at a partner fertility clinic, for a price quoted before treatment begins. You pay Sunfish for the program, and Sunfish pays the clinic. Sunfish collaborates with a member’s fertility clinic to support their care rather than replace it, and the member and their doctor make every medical decision.

The refund

  • The refund is one of the Gold Program’s benefits, alongside savings on long-term egg and embryo storage and transport, at-home wellness IVs, one-on-one mental health coaching, and more. Gold members get a guaranteed partial refund of up to $15,000¹, or a discount to continue treatment, if they are unsuccessful. Sunfish’s FAQs note that the exact amount depends on your package terms.
  • Sunfish prices each IVF Success Program from a predictive cost forecast, and returns a partial refund of up to $15,000, or a discount to continue treatment, if that forecast proves inaccurate.¹ See how Sunfish’s guaranteed refund works.

What sits outside the package price

  • Medication and PGT-A genetic testing are outside the package price. Medication comes through Sunfish’s partner pharmacy with a guaranteed lowest-price match. The clinic bills PGT-A if you choose it. Sunfish names both costs at the quote.
  • The program terms state: “Additional costs may be incurred outside of the Treatment Package Costs.”

What “all embryo transfers included” means

Sunfish’s IVF Success Program includes all single embryo transfers of a patient’s eligible embryos at one fixed price for up to a year,² in place of the separate fee a fertility clinic charges for each frozen embryo transfer. So if a transfer doesn’t work, the next attempt is already paid for.

Who can join, and where

  • The program is offered only through Sunfish partner clinics.
  • A physical evaluation at an eligible IVF clinic is required.
  • There are no additional qualification requirements for the IVF Gold Program. See IVF Success Program qualifications.
  • The Sunfish IVF Success Program Agreement governs the details.

Ask a Planning Partner any questions, and read the Agreement before you enroll.

Main takeaway

A refund promise is only as useful as its terms: before you pay, read how success is defined, what’s excluded, and when money comes back. Hold every program to that standard, including ours.

Sunfish brings financial protection, cost transparency, and personalized support together in one program, with its key terms in writing up front. When you’re ready, explore the IVF Success Program or get started. Terms and conditions apply.

Frequently asked questions

What are the disadvantages of IVF refund guarantee programs?

Most sit in the terms, such as how success is defined, what counts as a cycle, conditions that can end eligibility, costs outside the price, partial refunds, and time limits. The ASRM Ethics Committee also notes that people who succeed early may pay more than under per-cycle pricing.

What is usually excluded from an IVF refund guarantee program?

Medication and screening are commonly excluded. Genetic testing, embryo storage, and care for complications may be excluded too. Check the program’s written list before you pay.

What can end your refund eligibility partway through treatment?

In many programs, withdrawing early, skipping a required step, or a medical change can end your refund eligibility. The rules differ from program to program, so read the agreement closely.

Is the refund usually full or partial, and how is the amount worked out?

It’s often partial. Many programs work out the refund from the program fee, not from everything you spent. Ask for the exact formula and a worked example.

Does a refund program guarantee a baby?

No. A refund program defines when money may come back. It does not guarantee pregnancy or a live birth. That’s true of Sunfish too: financial protection is a program benefit, not a medical guarantee.

When does the Sunfish refund apply, and what is the most you can get back?

The refund is on the Gold Program only. Gold members get a guaranteed partial refund of up to $15,000¹, or a discount to continue treatment. It applies if a live birth isn’t achieved within your Custom Financial Plan budget and there were unexpected costs the planning tool didn’t predict. The exact amount depends on your package terms, and the Sunfish IVF Success Program Agreement governs the details.

¹ As part of the Sunfish IVF Success Program, Sunfish provides a Custom Financial Plan using Sunfish’s proprietary software that calculates the expected financial costs for each family to achieve a Live Birth based on a range of factors. If you fail to achieve a Live Birth within your custom financial plan budget and there are unexpected financial costs that were not accurately predicted by the Financial Planning Tool, our software Warranty may entitle you to receive a partial refund of your treatment package costs. Additional costs may be incurred outside of the Treatment Package Costs. See our Sunfish IVF Success Program Agreement for conditions and details.
